The Carmel Fire Department Mobile Integrated Health Care Program has begun a new follow-up program to check in on recently released hospital patients to check vital signs, make sure they are taking their medication and look for potential hazards like mold in the home or tricky stairs.

The paramedics have administered vaccines, drawn blood for tests, and have driven patients to the pharmacy or lcal clinic rather than the emergency room.

The goal is to cut down on the number of 911 calls that ten to be made withing 30 days of a patient's release, and to subsequently save the expenses incurred by patients, hospitals and insurers when a patient has to be re-admitted.
